Housing Case Manager

Salary: $60,000 - $70,000 per year, plus benefits and up to a $5,000 sign-on bonus based on the satisfactory completion of the three month probationary period.

Stable, dignified housing enables families to prosper, youth to excel in school, and neighborhoods to realize enhanced vitality. La Colaborativa is looking for dedicated case managers who will work with individuals and families to craft and execute goal-oriented service plans, resulting in increased housing stability, health, and wellbeing. This position will operate in a manner that is trauma informed, human centered, and deeply compassionate. Tasked with housing case management and systems navigation, the Case Manager will function as a vital link between families, resources, and opportunities. Working independently and as part of a team of human services professionals, this position will play a critical role in affecting the short- and long-term economic and housing stability families facing homelessness.

Key Responsibilities

  • Bilingual Spanish-English
  • Perform intakes, collect and analyze data, and conduct needs assessments to determine short and long term housing, economic stability, social, and health needs;
  • Under the leadership of the Triage Manager, assess housing barriers experienced by clients, while developing strategies to assist clients in overcoming barriers to obtaining stable, safe, and affordable housing;
  • Develop and implement trauma informed, culturally sensitive housing and wrap around service delivery plans;
  • Provide case management to a caseload of approximately 15-20 clients. Clients may reside in unstable housing conditions, local emergency shelter, or may be unhoused. Exercising empathy, empower clients through case management and advocacy to help clients achieve housing goals;
  • Offer resource navigation assistance to clients, in order to increase access to a variety of housing resources, public benefits, and community services, such as local rental and utility assistance;
  • Through a client centered approach, develop and implement strategies to maximize the impact of housing resources. Conduct extensive research and housing search activities to identify new housing opportunities for clients, supporting all aspects of rehousing;
  • Manage cases independently, as well as part of internal teams and external coalitions, in conjunction with community based partners;
  • Foster and maintain robust partnerships with community based organizations active in the housing field, in order to enhance the quality of service for clients;
  • Empathetically resolve conflict, promote social cohesion and wellbeing, and connect clients to integrative mental health, physical health, and specialized community-based services;
  • Advise leadership on emerging trends related to housing insecurity, displacement, and evictions;
  • Carry out all responsibilities with the utmost integrity and honesty; and
  • Perform data collection, prepare regular reports, and routinely update leadership in a timely manner; and
  • Prioritize cultural safety and sensitivity, specifically with Latinx and immigrant communities, in all facets of work.

Qualifications and Skills

  • Bilingual English-Spanish
  • High School Diploma or GED, Bachelors Degree Preferred;
  • 1+ years of customer service experience, preferably in human services, possess demonstrated commitment to customer service;
  • Must be over 21 years old;
  • Demonstrable track record of working independently and dependably completing assignments;
  • Highly skilled at written and oral communication, as well as tactfully interacting with people through a trauma-informed, culturally sensitive approach;
  • Excellent ability to carry out responsibilities independently as well as collaboratively as a team;
  • Capable of being adaptable, agile, and proactive;
  • Detail oriented, methodical, and organized, with excellent time management and organizational skills;
  • Experience working in an innovative manner, tackling challenges and assignments in unconventional ways or on tasks that require creativity;
  • Background in excelling in a fast-paced, dynamic environment. Experienced at creating and implementing systems to remain organized and complete tasks on-time;
  • Empathetic and compassionate, with a strong sense of community.

Environment

This position is a full-time, in-person position physically located at 63 Sixth St. in Chelsea, MA in a brand new, state-of-the-art headquarters facility with collaborative work areas, private offices, communal kitchens, and an outdoor plaza that enhances connections between team members. As this position works closely with community members, no remote work is permitted. Applicants must possess adequate transportation to commute to and from work.
